If you are a Kentucky resident and wish to play for a USYS club in another state, you must complete the Out-of-State Registration Form. This is a mandatory requirement to ensure your eligibility and compliance with KYSA and USYS regulations.
Out of State Registration Fee: $35*
Once the out of state player registration has been completed, information will be forwarded to the State association listed on the form in which your son/daughter will be playing alerting them of the registration. This process is now entirely online, there will not be a form sent to the parent as done in years prior.
Players who play with a Kentucky Youth Soccer member club but reside in a different state, must register with the State Association where they live. Information on those processes can be found at the respective state links below:

Permission to Travel: Teams traveling to USYS Sanctioned Tournaments and events outside of the Midwest Regional MUST submit a Permission to Travel Request via GotSport prior to traveling/participating. The Midwest Region includes: Kentucky, Ohio, Michigan, Indiana, Illinois, Wisconsin, Minnesota, Iowa, Missouri, Kansas, Nebraska, North Dakota, and South Dakota.
Participating in an in-state (Kentucky) sanctioned event DOES NOT require a permission to travel request. If you are participating in an event within the Midwest Region, you are permitted to use your State Approved Roster as your verified out-of-state permission.
Guest Player Form: Players on an official Kentucky Youth Soccer roster guest playing for another KYSA registered team are required to submit the Guest Player Form via GotSport before participating in a sanctioned event. Guest Player Forms MUST be submitted at least 5 business days prior to the start of the event and will be approved within 48 hours of the event.
Please note: Guest Player Forms must be approved by the State Office before a guest player is deemed eligible. Players will be notified of eligibility via GotSport.
DO NOT use the Guest Player Form for KPL Games. The Kentucky Premier League has a separate Club Pass process – learn more below!
USYSA Interstate Permission Form: Players on an official Kentucky Youth Soccer roster who wish to play as guest players with a team from another state association must complete the USYSA Interstate Permission Form. Once complete, please email to lisaduvall@kysoccer.net.
Permission to Host Notification (Word): Used anytime a team is hosting games for teams that are not member of Kentucky Youth Soccer but are members of another US Soccer Federation member. Once complete, please email to lisaduvall@kysoccer.net.
Notification to Play Games Against Non-KYSA Teams (Word): Used anytime a team is playing against teams that are not members of Kentucky Youth Soccer or any US Soccer Federation member. Once complete, please email to lisaduvall@kysoccer.net.
